Reduced visibility prevented the captain of a Jazz Aviation de Havilland DHC-8-300 Dash 8 and the driver of a fuel tanker at Toronto/Lester B. Pearson International Airport (photo shows overview of airport) from averting a taxi lane collision that prompted a chaotic evacuation of the turboprop aircraft, the Transportation Safety Board of Canada (TSB) says.
Fourteen passengers and the captain were injured by the impact and the subsequent evacuation, during which some passengers ignored the flight attendant’s instructions, opened emergency exits without authorization and tried to take their luggage with them as they exited the aircraft, the TSB said in its final report on the May 10, 2019, accident, released in early September.
The report cited the limited field of view from the fuel tanker’s cab and the captain’s limited visibility due to darkness, rain and reflected light as conditions that “caused or contributed to” the occurrence.
