Missing Safety Lockwire Cited in 767 Fire

The failure by maintenance personnel to install a safety lockwire on a fuel line coupling led to the Oct. 29, 2015, fuel leak and fire on a Boeing 767-200ER at Fort Lauderdale-Hollywood (Florida, U.S.) International Airport that seriously injured one passenger and caused substantial fire damage to the airplane, the U.S. National Transportation Safety Board (NTSB) says.

In its final report on the accident, issued June 8, the NTSB said that the accident occurred as the Dynamic International Airways charter flight, with 101 passengers and crew, was about to begin a scheduled flight to Caracas, Venezuela, operating under U.S. Federal Aviation Regulations Part 121.

Before the 767 was pushed back from the gate, the crew had noted an anomaly with a fuel quantity indication and fuel tank configuration that had been set by mechanics, the NTSB report said.
