Fatigue Cited in Fatal 2018 EMS Crash

The two medical crewmembers on the Eurocopter AS350 B2 repeatedly shouted the pilot’s name but got no response as the helicopter descended in a steepening right bank, rolled inverted and crashed to the ground near Hazelhurst, Wisconsin, the U.S. National Transportation Safety Board (NTSB) said.

The crash, during the late-night repositioning flight of the emergency medical services (EMS) helicopter on April 26, 2018, killed all three crewmembers and destroyed the helicopter, the NTSB said in its final report on the accident.

The probable cause, the report said, was “the pilot’s loss of helicopter control as a result of fatigue during cruise flight at night.”

Earlier in the evening, the helicopter had departed around 1759 local time from the Howard Young Medical Center Heliport in Woodruff, Wisconsin, on a series of flights, including two patient transport flights and one flight to Dane County Regional Airport for refueling.
