The primary purpose of compiling this list of safety issues was simply to focus attention on concerns that are still commonly held across the aviation industry. Additionally, we wanted to bring to the surface some safety issues of current concern to our readers and Flight Safety Foundation members, or that may not get enough attention.
To develop this “Top 10 Safety Issues” article, we first solicited input through FSF membership rolls. We received responses from risk-management specialists and others representing large international air carriers, airline trade associations, regulators, business aircraft operators and other industry stakeholders; and a number of individual members who are pilots, consultants and other types of safety or operational professionals.
Some entities sent us their internal lists of safety issues based on their own data; others took it upon themselves to informally survey their colleagues on our behalf; and some respondents simply sent us their own opini…
